8/24/2023 EnchantaVaughn Approved (AM) Upon arrival to the school setting, client displayed a polite demeanor and smiled when greeted by mental health professional. Client said good morning and was observed preparing for his morning work. Client was focused and on task. Client appeared happy as evidenced by him smiling and laughing. Client presented himself with good hygiene and clean appearance. Client had appropriate interaction with peers. Client was having appropriate conversation. Client was respectful of staff and peers. Client was helpful to his peers by sharing and playing appropriately in class. His teacher reported that this is the most that she has ever seen client behave and focus in class. His teacher hopes he continues this behavior and motivate others that struggle with staying seated and working alone to get their work done too. Client was courteous to authority requests and demands. (PM) Client engaged in group discussion utilizing team building skills and active listening skills. Client expressed positive thoughts while still showing interest in his treatment. Client was excited to attend his psycho social and joined his group. Client continued to treat others with respect and adhere to school rules. Client is seen focused and on task. Client negative symptoms have stabilized, he has begun to cope with all stressors, as well as, decreasing his impulsive behaviors while in the class setting. Client is beginning to process information that is important to his education and this has significantly improved his relationships at school. (EOD)Client implements relaxation techniques by taking deep breaths and counting to 10 when he feels overwhelmed and continues to adapt to his social environment. Client had a good day following directions, staying on task. Client was provided with feedback about how his behavior sets the tone for his mental stressors. Client seemed to be enjoying himself and getting along with others. Client was supported with achieving his objectives.
